The birth or adoption of a child is considered a Qualifying Life Event (QLE), which allows you to update your benefits within 30 days of the event.

Free prenatal vitamins can help you baby a healthy start.

Through SynchronyRx@Home, eligible employees can receive prenatal vitamins at no cost.

How to Receive Vitamins

1. Enroll with SynchronyRx@Home.
2. Ask your doctor for a prescription for prenatal vitamins.
3. Submit the prescription to SynchronyRx@Home.

SynchronyRx will determine the brand provided.

Employees may receive up to:

• Two maternity uniform polos, or
• Two maternity chef coats

Eligible full-time and part-time employees who have worked at Trilogy for at least six consecutive months may qualify for Paid Parental Leave following a birth or adoption.

Important Information

• Available for both birth and adoption
• Leave must be taken when your child arrives
• Leave may be required to run concurrently with FMLA or other applicable leave programs
• Requests should be submitted at least 30 days before your anticipated leave date whenever possible
• If both parents work for Trilogy, they do not need to take leave at the same time
• Multiple births or adoptions do not increase the total leave available
• Approval is subject to eligibility requirements and company approval
